In order to submit the Address Change Form you will need to go to the main search page located HERE and search for your property.
Once you’ve found your property and selected it, it will open a Property Report Page, click on the Address Change Form from the top menu.
This button will not open the Address Change Form; you will need to open the one on the Property Report Page under Applications & Forms.
Once you click on the Address Change Form button, it will automatically fill out part of the form for you. Please fill out the remaining information and click Submit.
